Your custom field name had a space in it, which does not work. This would cause errors and keep functions like the email from working. We have corrected this on your site. Please verify that it is working correctly now. Thanks.

Verified, everything is working fine now. Thank you Nathan. I would just add that you should update the component so that when you hover over the blue info icon that it specifies that you can't have any spaces (some of us really do read those).
