We have a few records where, in backend, the mandatory fields (including firstname, lastname, etc) are not populated. We have the latest version of DT Register 3.1.11b , and is happening with a random basis.

When this happens, does the record contain other data and just missing the name? Or is everything missing? We'd have to see this.

The list of records is hard-coded to use the default installed "firstname" and "lastname" fields. It sounds like you are using different fields that you have created. Am I correct? Those won't show in the records list view without a code modification.

No, that is not the case.

(1) For the first-name and last-name, we are using the default ones that came with the extension.

(2) It is not about just on the list view. Those missing fields are not there at all in the system. Neither in the detail view nor in the csv export, nor in the database.

(3) And it is random. Some records have all the good data, and some have missing names, and some have missing custom mandatory fields.

Then you are losing session data. You need to be running 3.1.11c as there was a session related fix put in place... basically an added check that will rebuild the session if it is not complete or lost. The issue happens almost entirely in the Chrome browser. Update though if you haven't yet. If the issue continues, submit a ticket.
